After a ton of research and troubleshooting I believe I have at least discovered all of the root causes. This is my solution to one of them. One of the problems I was seeing was that the credential permissions on the records that were created via the Microsoft dynamic DNS process were hosed up. After some Sherlock Holmes style sleuthing I managed to find a pattern.
All of the servers for these records were re-imaged around the same time. It turns out whenever a computer is brought onto a domain and registers its DNS record, re-imaged or the OS is just reinstalled without removing the DNS record nor removing the AD computer account as part of the process problems can crop up.

One of DC's is now a primary and the other is tertriary. The non-AD server the one that has mismatched dns records is the secondary. On the primary DNS server, right-click the zone that you want replicated and choose Properties. Click the Zone Transfers tab and verify that you have that setup correctly: If the secondary DNS server is listed in the Name Servers tab, then select the option here for "Only to servers listed on the Name Servers tab. After you do this, click OK.
This should force a zone transfer of only the changes since the sec. If that doesn't work, then right-click the zone again and choose Reload from Master. This will force a zone transfer of everything on the primary to the secondary. Also, make sure that the secondary zone still points to the primary DNS server.
You implied that many changes have occurred I'm wondering if some kind of misconfiguration in terms of where the server is trying to pull from is the cause. Let me know if that helps. Worst case scenario is you having to deleting the secondary zone and recreating it since it's just a secondary zone, it'll be fine since it'll just be pulling those records again from the primary zone.

I guess that the forward lookup records are created by the host, but the reverse records are created by the DHCP server? Why is that? It seems pretty arbitrary. What OS are the hosts? Are ANY reverse lookup records being created without your manual intervention? The hosts are windows.
